Topiramate and Weight Loss: Off-Label Use, Qsymia and Safety
Topiramate is approved for certain seizure disorders and migraine prevention—not as a stand-alone weight-loss drug. Clinicians sometimes prescribe it off label because reduced appetite and weight loss can occur. Qsymia is a distinct FDA-approved extended-release combination of phentermine and topiramate.

Off-label prescribing
Off-label use can be lawful and clinically appropriate, but it requires informed discussion of evidence, alternatives, uncertainties and risks. It should not be represented as an FDA-approved topiramate weight-loss indication.

A clinician may consider medical history, migraine or seizure indications, eating patterns, prior response, cost and the suitability of approved alternatives.
Topiramate versus Qsymia
Qsymia has its own fixed-dose extended-release formulation, escalation, pregnancy precautions and stopping rules. It is not equivalent to casually combining separate tablets.

- A complete review of weight history, previous treatment and current goals
- Screening of blood pressure, medical conditions, pregnancy plans and medication interactions
- Discussion of expected benefit, side effects, insurance coverage, availability and cost

Important safety considerations
- Topiramate can cause fetal harm, including oral clefts; pregnancy prevention and testing requirements must be addressed when relevant.
- Cognitive slowing, word-finding difficulty, mood changes, tingling, taste changes and fatigue may occur.
- Risks include metabolic acidosis, kidney stones, acute eye problems, reduced sweating/heat illness and elevated ammonia in certain combinations.
- Do not stop suddenly when seizure risk is relevant; tapering must follow clinical guidance.
Emergency warning: Seek urgent care for severe allergic symptoms, chest pain, fainting, severe or persistent abdominal pain, inability to keep fluids down, signs of dehydration, or other potentially serious symptoms.
