Qsymia and Phentermine/Topiramate for Weight Loss
Qsymia combines phentermine with extended-release topiramate in fixed strengths for chronic weight management. It requires careful pregnancy prevention, dose escalation and response monitoring.

What patients should know
Qsymia affects appetite through two active ingredients. It is not interchangeable with taking arbitrary doses of separate phentermine and immediate-release topiramate. Product formulation, titration and monitoring follow the approved label.
The label includes a REMS because fetal exposure to topiramate is associated with increased risk of oral clefts. Patients who can become pregnant need pregnancy testing and effective contraception according to current requirements.
Who may be evaluated
- Eligible adults and labeled adolescent populations after medical evaluation
- Patients preferring an oral chronic-treatment option
- People who can comply with pregnancy precautions when applicable
- Patients without glaucoma, hyperthyroidism, MAOI use or other contraindications
Potential advantages
- Once-daily oral formulation
- Chronic weight-management indication
- Multiple dose strengths
- Defined response checkpoints in labeling
At-a-glance comparison
| Treatment | Formulation | Main difference |
|---|---|---|
| Qsymia | Fixed phentermine/topiramate extended release | Approved chronic-treatment product with defined dosing |
| Separate phentermine plus topiramate | Individual products | Off-label combination is not formulation-equivalent |
| Phentermine alone | Single ingredient | Labeled as short-term adjunct |
Safety and medical supervision
- Do not use during pregnancy; follow testing and contraception requirements.
- Important concerns include increased heart rate, mood changes, suicidal thoughts, cognitive effects, metabolic acidosis, kidney stones and eye problems.
- Dose tapering may be required when discontinuing the highest dose to reduce seizure risk.
- Renal or hepatic impairment may affect dose limits.

Frequently asked questions
Why are pregnancy tests important?
Topiramate exposure in early pregnancy is associated with oral-cleft risk, and Qsymia has specific pregnancy-prevention requirements.
When is response assessed?
The prescribing information uses weight-loss checkpoints after specified periods at treatment doses to guide escalation or discontinuation.
Can Qsymia be stopped suddenly?
The highest dose should be discontinued gradually according to prescriber instructions because abrupt topiramate withdrawal can precipitate seizures.
